Client Intake
Please fill out the client intake form prior to your initial session. This form includes important disclaimers that will require some initials, as well as a section for your signature. Additionally, there is a dedicated page designed to help better understand you and where your are at presently. This will also help to provide the proper approach to suit you best. Rest assured, your responses will be kept confidential and will only be used to improve your experience with SereenSolutions.

Relationship Assessment
This assessment only needs to be filled out if you are taking advantage of the Nurture Connections Service and are currently in some form of a relationship. It is designed to measure your relationship satisfaction. Both you and your partner can fill this out, submitting it individually, or if we are starting with you then only one submission is needed. If you are single and signing up for Nurture Connections to enhance your goals for future relationships, you do not need to submit. This is a scale rating questionnaire, simply formatted with only seven questions.
